Registration for SUMMER 2013 is now open! Summer Camp runs the week of July 29-August 2nd

Be sure to fill in all the required information. After you complete the registration form you will be prompted for a $20 non-refundable deposit to reserve a spot in the class.

Medical Consent and Release of Liability (please initial)

I, the undersigned parent or guardian of this student, a minor, do hereby authorize the directors and teachers of Northwest Youth Theatre (NYT) as agents for the undersigned to consent to medical treatment in an emergency. I hereby release and discharge NYT from any and all claims for personal injuries.

I agree that photographs of this student taken during workshop hours may be used for promotional purposes by NYT.

Payment Information

Grades 1-3: Summer Camp Tuition for It's a Jungle Out There! is $135.00, due in full on the first day of camp. Receive a $15.00 discount when you register AND pay in full by June 1st.

Grades 4-8: Summer Camp Tuition for Radio Hour (and Drama Workshops) is $180.00, due in full on the first day of camp. Receive a $30.00 discount when you register AND pay in full by June 1st.

To secure enrollment in any camp session, please send non-refundable $20 deposit for each camp session. The remaining balance for camp tuition will be due the first day of class. On the following page you will also have the option to pay in full or submit a deposit only.

REFUND POLICY: Full refund if class is canceled by NYT. Tuition, less deposit, will be refunded up to second class session.
